Community Care Packages

Community Aged Care Packages provided by MCC are a planned and coordinated package of support to assist a person who requires management of services because of their complex care needs. The packages are targeted at frail older people living in the community, with particular focus being for people from culturally and linguistically diverse backgrounds. People accessing a package would otherwise be eligible for at least low residential care.

Services available through the packages include:

• personal care such as showering, dressing and hygiene support),
• home help , which includes culturally appropriate cooking, domestic duties and laundry,
• social support including chatting, transporting to doctors and specialist appointments, community
• inclusion and supporting daily needs such as banking and shopping.

24 hours emergency contact is available to clients and carers.

As well advocacy is an integral part of this package of care.
