If you are a frail older person and you need assistance, you may be eligible for some services. Eligibility is targeted to people with complex care needs who want to remain living at home. You will need to be approved for a community aged care package by the Aged Care Assessment Team (ACAT), which we can organise. Our focus is targeted to people who are from a culturally or linguistically diverse background.
Home and Community Care (HACC)
If you are a frail older person or a person of any age with a disability or a carer, and you are from a culturally or linguistically diverse background and need some support to remain at home you may be eligible to receive services.
Centre Based Day Respite
If you are a frail older person or have a disability and you are from a culturally or linguistically diverse background and are socially isolated you may be eligible to attend our centre based day respite.
Community Visitors Scheme
If you are an older person in a nursing home wanting someone from a culturally and linguistically diverse background as a support person, you are eligible to access our services.
Overnight Respite
If you are a carer and you need support in your caring role, we may be able to help. People who are carers supporting a person who is frail aged, or a person who has dementia, or a person who has dementia with challenging behaviour, or a person who has a terminal illness in need of palliative care, and you or they are from a culturally and linguistically diverse background, we may be able to provide support services.
